Making a claim for an individual policy

You can send your documents to our head office at the address below or submit them at your nearest QNB Bank branch. For inquiries, please get in touch via our hotline on 19753.

Submitting a death claim

Please include the following documents of the insured:

  • Original death certificate.
  • Original medical report stating the cause of death.
  • Original policy.
  • Copy of the insured’s National ID.
  • Original police report along with the prosecution’s final decision in case of accidental death.

If the beneficiaries are minors (less than 21 years old), please include:

  • Original custodianship certificate.
  • Copy of beneficiaries’ identification documents.
  • National IDs for beneficiaries and birth certificates for beneficiaries younger than 15 years of age.

Submitting a disability claim

Disability cases are to be referred to one of the company's medical consultants for assessment. Please include the following documents of the insured:

  • All medical reports related to the disability condition, including the original medical report clarifying the onset and cause of disability, and the original medical report confirming that the disability persists after six months.
  • Full medical file.
  • Original policy.
  • Copy of the insured’s National ID.
  • Original police report along with the prosecution’s final decision in case of accidental disability.

Making a claim for a corporate policy

For corporate policies: kindly refer to your company HR department. If you are from the HR department and want to submit a claim for any of the company’s insured employees, kindly submit the following documents to QNB Life Insurance head office at 5–7 Gezerit El-Arab Street, Star Capital Building, El-Mohandessin or to your nearest QNB branch.

Death claim:

  • Original death certificate.
  • Original medical report stating the cause of death.
  • Copy of insured’s National ID.
  • In case of accidental death: original police report along with the prosecution's final decision.
  • Full medical file.
  • Letter for the insured sick leaves during coverage period.

Disability claim: disability cases are to be referred to one of the company’s medical consultants to assess disability.

  • All medical reports related to the disability condition including original medical report clarifying the onset & cause of disability and original medical report confirming the disability persistence after 6 months.
  • Full medical file.
  • Copy of insured’s National ID.
  • Letter for the insured sick leaves during coverage period.
  • Letter for the insured sick leaves during coverage period.
  • Governmental disability confirmation if available.

  • If beneficiaries are the legal heirs: original succession certificate. If beneficiaries are nominated, then no need to submit the original succession certificate.
  • If beneficiaries are minors (less than 21 years): original custodianship certificate.
  • Copy of beneficiaries’ identification documents National IDs for beneficiaries older than 15 years old and birth certificates for beneficiaries younger than 15 years old.
